Quaker Valley High School Marching Band Temporary Emergency Waiver Form–to be filled out, signed, and sent to Nurse Gilmore at gilmorea@qvsd.org (preferred), or presented in person at the first rehearsal.  Only students who must carry their own medication and/or have medication administered to them by the Registered Nurse at Marching Band events must complete this form ONLY IF they are temporarily unable to produce the QVHS Medication Administration Request Form before the first rehearsal.
