Marching band medical form required of all student participants–to be filled out, signed and sent to Nurse Gilmore at gilmorea@qvsd.org (preferred), or presented in person at the first rehearsal. No student may participate without this completed document.

Quaker Valley High School Medication Administration Request Form–to be filled out, signed, and sent to Nurse Gilmore at gilmorea@qvsd.org (preferred), or presented in person at the first rehearsal.  Only students who must carry their own medication and/or have medication administered to them by the Registered Nurse at Marching Band events must complete this form.

Quaker Valley High School Marching Band Temporary Emergency Waiver Form–to be filled out, signed, and sent to Nurse Gilmore at gilmorea@qvsd.org (preferred), or presented in person at the first rehearsal.  Only students who must carry their own medication and/or have medication administered to them by the Registered Nurse at Marching Band events must complete this form ONLY IF they are for some reason unable to produce the QVHS Medication Administration Request Form (above) before the first rehearsal.
